Is an Online Evaluation the Same as Applying for the State Card?
No. The physician evaluation and the Georgia state card process are separate.
The online evaluation allows a Georgia physician to review your eligibility. When the physician decides to certify a patient, the physician completes the Medical Cannabis Certification and submits the required patient and caregiver information to the Medical Cannabis Patient Registry.
Georgia DPH then verifies the submitted information. If the submission is accurate and complete, DPH emails the patient or caregiver instructions for paying for the state-issued card.
Who Can Schedule a Georgia Medical Cannabis Evaluation?
GMMC’s evaluation service is intended for Georgia residents who have, or believe they may have, one of the state’s qualifying medical conditions.
Certification must come from a licensed Georgia physician with a valid Georgia license and principal practice in the state, and the evaluation must be completed by a licensed physician.
Before scheduling, review the Georgia medical cannabis qualifying conditions. Some conditions have additional age, stage, severity, diagnosis, or trauma-exposure requirements.
Examples include:
Georgia recognizes intractable pain, not chronic pain in general.
PTSD applies when the patient is at least 18 and the condition results from direct exposure to or witnessing a trauma.
Peripheral neuropathy symptoms must be diagnosed as severe.
Autism has different wording for adult and minor patients.
sickle cell disease is also recognized on Georgia’s qualifying condition list.
amyotrophic lateral sclerosis is included among the qualifying conditions recognized by Georgia.
Having a listed diagnosis does not automatically require the physician to provide certification for a marijuana card in Georgia.

Not every patient will need the same records. The physician may request additional information based on the condition and individual circumstances, so review Georgia’s qualifying conditions before your visit. Use DPH's current qualifying-condition list rather than an older condition count.
Providing documents does not guarantee certification. The records help the physician understand your diagnosis, history, and current condition. Examples of qualifying conditions include cancer, PTSD, crohn's disease, a form of inflammatory bowel disease, AIDS, and multiple sclerosis, which are among the Georgia medical marijuana qualifying conditions. Patients in a hospice program may also qualify under Georgia law. Georgia's registry includes minors and caregivers, but GMMC provides adult consultations only. 3. Discuss your medical history and condition
The consultation is conducted by one of GMMC’s licensed doctors, who may ask about:
Your diagnosis
Current symptoms
How long you have had the condition
Medical records or test results
Current medications
Previous and current treatments
Relevant caregiver circumstances
Answer accurately and completely. Do not stop medication or change treatment solely because you are seeking a Medical Cannabis evaluation.
4. Receive the physician’s decision
The physician determines whether certification is medically appropriate based on the evaluation and professional medical judgment.
Certification is not guaranteed, and it is not the same as a prescription. Under Georgia law, that certification decision is made by a licensed medical marijuana doctor. Georgia DPH describes certification as the physician’s acknowledgment that the patient has an established physician-patient relationship and one or more conditions recognized under Georgia law.

How Long Does the Process Take?

How Long Is the Georgia Medical Cannabis Card Valid?
The physical Georgia Medical Cannabis card is valid for five years. The expiration date appears on the front of the card.
DPH's physician guidance describes the general annual Medical Cannabis Certification process. Georgia law exempts patients with incurable or irreversible conditions from annual certification.
